TYPES AND SYMPTOMS

Piles are swollen or thrombosed blood vessels inside or outside anus.

Internal piles are generally bleeding in nature and are painless, with patient complaining of drops of blood during stools.

External piles are painful and feel like a swelling or a bunch of nodules. They are generally non bleeding. External piles are felt as swelling outside the anus and are tender to touch as they may be thrombosed, this may cause spasm of anus causing retention of stools and increasing their bulk and hence exacerbating the problem by entering a vicious cycle.

Sentinel pile is actually a dead skin tag that hangs out after been torn from the rectal mucosal lining.

Othe symptoms of haemorrhoids may be

  • Itching around anus.
  • Feeling of lump in anus while sitting.
  • Feeling something hanging.
  • Pain as if something was sticking inside anus or like a thorn stuck there.

PRECIPITATING FACTORS

  • Sedentary life style- long sitting hours either due to work or habit may predispose a person to develop piles.
  • Eating non healthy and processed food.
  • Conditions that increase intrabdominal pressure such as Pregnancy.
  • Obesity.
  • Medication causing constipation
  • Compromised liver etc.

DIET

Dietary fibres are important part of a balanced diet.

They give bulk to the stool and ensures gut health by enhancing growth of beneficial flora, promotes gut movement and binds toxins.

Lack of fibre in the diet causes constipation, which in turn pushes the person to apply pressure during stools.

A chronic tendency of such habit can cause both external and internal piles.

EXERCISE

A sedentary habit also promotes the development of piles.

Exercises ensures a regular bowel movement and helps in preventing constipation, which prevents pile affection.

STASIS DERMATITIS, VARICOSE VEINS AND ITS MANAGEMENT(HOMOEOPATHY)

What is stasis dermatitis?

Stasis dermatitis is hyperpigmentation and later scarring and destruction of the skin, especially of the lower extremities as a sequelae to venous insufficiency.

Who is more liable to have stasis dermatitis?

Found generally in old age but can be found in anyone where there’s damage to the venous system or its valves. Predilection to those with
Jobs requiring long standing,
Increased intrabdominal pressure from chronic constipation, tumour, pregnancy, obesity etc.

What causes stasis dermatitis?

The most common causes include
Varicose veins,
Harvesting veins for cardiac procedures,
Injuries to veins etc.

What are varicose veins?

One of the commonest causes of stasis dermatitis, varicose veins happen due to the incompetency of valves in the veins of the lower legs. The valves prevent the retrograde (backward) flow of blood. When they are incompetent the blood starts pooling in the legs, causing swelling, pain and pigmentation. Slowly the veins enlarge and become tortuous and prominent. As the skin starts getting affected, it becomes atrophic, pigmented and liable to varicose ulcers.

Complications of varicose veins

Complications include

Hyperpigmentation of skin

Scarring of skin, due to atrophic changes

Non-healing varicose ulcers- least injury to atrophic skin leads to varicose ulcers, the usual location of which is the inner side of lower leg (medial aspect just above ankle)

General Management

Regular exercising ensures that the blood flow in the extremities is maintained. Regular exercising also prevents heart diseases which in turn precludes venous insufficiency from harvesting veins for cardiac procedures.

Eating a fibrous and roughage-rich diet prevents constipation which is one of the reasons for raised intraabdominal pressure (which predisposes to varicose veins)

Raising the feet

This simple position while sitting, in those with predilection to varicose veins can in the long run delay the onset of varicose veins.

Massaging from below upwards

This simple massage practice improves flow of blood and prevents stagnation of blood in lower extremities.

Activating soleus

The calf muscles are called the second heart of the body. Simply moving your heels up and down while your toes are fixed while sitting or standing aids the flow of blood from lower extremities towards heart, preventing pooling of blood.

Weight loss
Losing weight helps in many ways in tackling varicose veins.
It improves cardiac function,
Reduces intrabdominal pressure
Improves flexibility and care of feet.

Stop smoking
Smoking causes oxidative damage to the vessels and may predispose to predominantly arterial blocking, but some recent evidence points to venous damage as well. In general, it is better to stop smoking for a better cardiovascular health.

Wearing compression stockings
Commercially and easily available in different sizes, compression stockings prevent pooling of the blood in the lower extremities. They sit tightly around the leg and may pose inconvenience to the wearer. Because of this reason their compliance is low. Nevertheless, when properly fitted and regularly used, they slow down the progress of the diseases to a great extent.
Caution- They are not be used during sleep.

Medical management

Homoeopathic medicine and
Giving constitutional medicine- this is would be given by your homeopathic physician after detailed case taking and taking many factors into consideration.
Massaging with HAMAMELIS Q- (I generally use HAMAMELIS Q 1 drop in 10 drops of oil i.e 1:10 ratio and ask the patient to massage feet from below upwards before sleeping).

HOMOEOPATHY AND MANAGEMENT OF STRESS IN GENERAL

Stress has become an integral part of modern life. Gone are the days when fulfilling the family’s daily needs was the breadwinner’s sole purpose. In today’s world, luxury is coveted, and we want to be a notch above everyone else. The stress of raising a family, handling diseases, work environment, travelling, finances, peer pressures, upward social mobility all are taking a toll on everyone’s health. Add to that the stress arising from gender expectations from centuries of norms for half of the population. It is very easy to get lost in such a maze of rat race.

Now, it’s neither easy nor practical to tell someone to become a saint and leave all the pleasures of life behind.

Nevertheless, you can constantly introspect, prioritize and act accordingly. There is always a way, a way that can be chosen if one is willing.

These following ways are what I advise my patients. They may or may not be fitting for you. Try to find your own way, something that is tailor-made to your needs. Test and try various methods, so that you enjoy the one life that is given to you, and make the most of it in any given circumstance.

Exercise to beat stress and elevate your mode

Exercise is scientifically proven to elevate mood, improve immunity and build strength.

The endorphins released after exercise improves your sense of well-being.

We, as humans, are not meant to sit, work, eat, sleep and lead a sedentary lifestyle. We are supposed to be on the move. Denying our evolutionary need is an invitation to trouble. So, get up and get running.

Any form of exercise that is suitable to you- walking, running, yoga, dancing, a mix of others; anything will work.

People who exercise into their 50s have higher bone mass than those who do not. If you want a good old age, start working on it from now.

Sleep to reduce stress naturally

Adequate sleep is essential for optimal functioning of the body.

You must have noticed the irritation and fogginess one experiences after a lost night’s sleep. Inadequate sleep increases your stress hormones and makes you jumpy while affecting your judgment (this could be risky if you were in a jungle and faced a lion; or while you were driving; or probably if you were a professional where urgent decision-making is the key- face that lion!).

During sleep, the mind processes, organizes and discards the information gained during the day.

Lack of sleep over a prolonged period of time, increases blood pressure, predisposes you to diabetes and deteriorates your mental powers.

So sleep into that night, tight. Maintain sleep hygiene (proper timings; dark environment, quiet surroundings etc.) and reduce your stress naturally.

Eat healthy food to keep stress away

It could be surprising to know we can eat healthy to keep stress away. 

Well, first of all, healthy food like greens, beans, seeds, fruits etc. contain antioxidants which combat the stress-related oxidative products of the body.

Secondly, what you eat affects your gut flora. There’s ample research and evidence coming to the forefront showing that chemical released by your gut flora affects your mood and the way you feel.

Thirdly, the antibiotics, hormones, pesticides and other chemicals regularly used to grow food are harmful to your body. It affects your own hormone axis, portends diseases, and creates chaos in your system. Hence its advisable to stick to organically grown food.

Find that balance

This is applicable to every walk of life if you truly want peace and want to keep your stress under check. Very few can be zen-like and still scale the heights of their professional and personal lives. For others, us mortals, it is close to fantasy, if not impossible.

Work, work hard but then find time for your kids, family, and friends. Find that balance.

Use your means to buy comforts of life for yourself, but find that balance. You don’t have to have the latest fashion or the latest car or the latest phone. Change when it’s needed. Not only will you be doing a favour to the environment, but you will find yourself standing on the sidelines of the rat race—one less thing to worry about.

This, by no means exclude those whose ambition is to be triumphant in their game. By all means, follow your dreams. The ones who loves their work, there’s not a day of work in life.

Keep your surroundings clean and green to beat stress

Keep your house/ your workstation clean and decluttered. There is no better way to destress than to enter a clean surrounding. Your mind is able to make much more meaning of clean surroundings.

Pots, planters, flowers etc., make one feel closer to nature. Add green around you. And there’s no therapy like nature therapy.

Plan yet be flexible

When you have multiple things to do, plan your day. This helps you prioritize your commitments. You may not be able to fulfil all, all the time, but it will give you a sense of control.

However, don’t be rigid, be flexible, for the world does not go according to one’s wishes.

Including that flexibility in one attitude helps you navigate those uncertain unforeseen turns in life. It is said that grass withstands the storm better than the mighty tree.

Talk and ask for help

There are times when the water crosses the threshold. When that happens, never hesitate to ask for help. Please talk about your problems, for sometimes, a solution is found when it is seen from a vantage point.

Be financially stable

It might sound a bit off place that you are getting a piece of advice on financial stability on a medical blog. But if you see the bigger picture, money and finances are a significant source of worry in today’s environment. Being prudent with money and making the right financial decisions can make your life easier and less stressful.

How homoeopathy helps beat stress

There are times when we cannot avoid the devil despite all our efforts. Such circumstances in themselves become maintaining causes for diseases and stymie our efforts to live a healthier life. HOW TO TAKE HOMEOPATHIC MEDICINES

GENERAL INSTRUCTIONS FOR TAKING HOMEOPATHIC MEDICINES

Homeopathic medicines are dispensed in different scales based on many factors. Depending on what the doctor has prescribed, the following are general directions for taking homeopathic medicines.

The common scales are:

TINCTURE
  • These are liquid homeopathic medicines.
  • They are to be mixed in ½ cup of water; the quantity mixed being as per directions of the physician. (10-30 drops)
  • They should be taken half hour before food- 2-3 times a day as per instructions.
  • For some patients in some conditions, it can be mixed in semisolid food and given, this is however an exception than a general rule.
DECIMAL SCALE
  • The homeopathic medicines of this scale are usually available in tablet form. (Some are in liquid for in which case the drop dosage of medicine should be taken in water before food)
  • They come in 3x/6x/ 12x potencies etc.
  • Generally, these are given 2/3/4 tabs twice or thrice daily, to be taken as per the advice of the physician.
  • These tablets can either be dissolved in hot water or taken directly, half an hour before a meal (empty stomach).
CENTESIMAL SCALE
  • Homeopathic medicines in this scale are usually given in dosage form.
  • The dose should be taken in the early morning after rinsing the mouth with water, without touching the medicine. After half an hour, teeth can be brushed and any other medicine is taken.
  • Some homeopathic medicines act better when taken at bedtime, in which case there should be a gap of 2-3 hr between dinner time and medicine intake.
  • In acute diseases, the medicine is given in pill form 3-4 pills 2-3 times daily according to the directions of the doctor. This should also be preferably given at least 15 min before food.
  • Sometimes the doctor may advise 4 pills of medicine to be dissolved in water and a spoonful taken every 15 min/ 30 min depending on the severity of the disease. The medicine dissolved and should be stirred nicely before giving a spoonful each time.
50 MILLESIMAL POTENCIES

These homeopathic medicines are dispensed in 30- 100 ml bottles.

Depending on what dosage the physician has advised, the whole bottle is to be taken in 30 equal dosages, generally. (A paper strip with equal 30 markings can be pasted on the side of the bottle so that only one dose is taken out each time without mistake).

  • Step 1- The bottle is given 10 hard jerks in downward directions to start with.
  • Step 2- Next, one dose of medicine is taken out and mixed in 6-8 spoonfuls (30-40ml) of water in a cup.
  • Step 3- The medicine is mixed thoroughly and one spoonful is taken from this cup each time (3-4 times per day or as per the physician’s directions).
  • Step 4-At the end of the day the left-over medicine if any is discarded and step 1 is repeated the next day again.

DIETARY RECOMMENDATIONS FOR PATIENTS WITH RENAL/ KIDNEY STONES

Kidney stones are a very common occurrence among the general population. Low intake of water and a tendency to form stones is one of the commonest causes of their formation.
